If a fax comes in partially, improperly or illegible it may not be listed here in this search. If your search does not produce the form you are searching for it may be due to one of the reasons listed below.

• Our system has not yet processed the form. This can apply to recently submitted forms.
• The form was received as a partial form due to an incomplete e-fax transmission.
• The form was received but was transmitted in one or more of the following conditions: upside down, with a cover page, with multiple (different) forms or an out of date form was used.
• The form was received but was illegible meaning that data could not be extracted from the form to allow for the search criteria to be matched.
• The form was not e-faxed. Please check with your local office.
